Registration Forms and Name Cards
¡¡¡¡ Every student must register for classes each semester. Registration includes enrolling in classes, the telephone registration system, paying fees and other financial obligations, filing (Ìá½»¡­ÒÔ±¸°¸) your current address with the Office of the Registrar (×¢²á°ì¹«ÊÒ), and providing other information forms.
 
¡¡¡¡When you register, you must:
¡¡¡¡Have a photo ID picture taken.
¡¡¡¡Submit a Statement of Legal Residence.
¡¡¡¡Return the completed Medical History form and the Insurance Information Request form. These forms are mailed to each new student by the Student Health Center.
¡¡¡¡Be sure to inform the Office of the Registrar of any change of your address, or your name. Keep your campus / local mailing address, your billing address and your permanent address up to date. Failure to file your current address can result in a delay of your registration (These are usually things required in the U.S.).

The Registration form is an official list or record containing a "summary" of related information about a person. Be sure to fill out all sections in full.
